NMJCA Physical Examination Form
This form must be completed by the applicant's primary care physician to certify their physical fitness for participation in the NMJCA program. It includes a full physical examination and clearance for strenuous activity.
NMJCA Power of Attorney Form
This document grants the NMJCA staff limited power of attorney to make decisions regarding your applicant's medical care, participation in program activities, and access to records. Required for program participation.
